Patients with Long Term Conditions

If you have a long-term condition (also known as a ‘Chronic Disease’) it is likely that you will need regular health checks (usually once or twice a year).arm-5

At Roman Road Health Centre we will write to you in the month of your birth to let you know which check-ups you need. Please follow the instructions in the letter.

If you have a long-term condition and do not receive a letter, please discuss this with your doctor.

We also have access to community nursing teams who specialise in certain conditions, for example, Diabetes, COPD, Heart Failure. Your doctor or nurse will let you know if you need to be referred to these teams.

Coronavirus – Advice for Patients

The symptoms are similar to other illnesses that are much more common, such as cold and flu and it does not necessarily mean you have the illness.
 
NHS 111 has an online coronavirus service that can tell you if you need medical help and advise you what to do.

Do not go to a GP surgery, pharmacy or hospital.

Like the common cold, coronavirus infection usually occurs through close contact with a person with novel coronavirus via cough and sneezes or hand contact. A person can also be infected by touching contaminated surfaces if they do not wash their hands.
The risk of being in close contact with a person with coronavirus or contaminated surfaces is very low at the current time.
