Registering With the Practice
You can register by clicking the following link and completing the On-line registration form or you can call into the surgery and collect a registration form. The practice aims to complete your registration within 3 working days on receipt of your completed registration forms.
Childrens Registration
If registering children under the age of 5 years :
Once the registration form has been completed/submitted, please also provide a copy of the child immunisation history from the red book to Roman Road Health Centre or email to repeats.p81709@nhs.net
